The Role
- Rota planning and scheduling to ensure consistent care delivery
- Creating and reviewing care plans and completing assessments
- Conducting staff appraisals and supporting care teams
- Liaising with service users, families and care staff
- Supporting day-to-day operational needs of the service
- Previous experience in a care coordination or similar role
- Strong organisational and communication skills
- Confident managing rotas, documentation and staff support
- Full UK driving licence required
